Making a Referral

We aim to provide a straight-forward path for referral from community physicians and nurse practitioners to allow us to participate in the care of your pregnant patients.

Self-referral or a standard referral letter from your primary care provider are both acceptable.

Pinnacle Family Medicine Obstetrics can be found on Ocean to facilitate your electronic referrals

Referrals can be directed to a specific provider (for example, the physician who delivered a previous child), a specific location or clinic, or generally to our physician team.

We generally will expect to assume care around 24-28 weeks of pregnancy, but are happy to receive referrals at an earlier gestational age.

Our referral form can be accessed as a PDF then completed and faxed to our central booking system at New Vision Family Health Team.
